Parents can download these application forms as and when required, fill it with relevant information and bring the same to school office.

Application Form for Issuance of Duplicate I-Card

The application form for issuance of Duplicate I-Card may be downloaded by the parent, filled in and submitted at the School Office with the following requisite fees:

Parent’s copy of I-Card : Rs. 500.00

Student’s copy of I-Card : Rs. 500.00

Administrative Charge : Rs. 200.00

New I-Cards will necessarily have to be issued in case of change in address/telephone number or mode of transport, on payment of the above mentioned charges.

Application Form for Issuace of Bonafide Student/Fee Certificate

The application form for the issuance of Fee Certificate, which is generally sought by parents for ITR purposes or getting the education fee reimbursed by their employers, may be downloaded by the parent, filled in and submitted at the School Office. The same shall be issued if and only if all dues of the concerned parent are clear.

Bonafide Certificate Charge : Rs. 100.00

Application for Change in Address

The application form for Change of Address/Phone Nos. may be downloaded by the parent, filled in and submitted at the School Office for updation in the School records with the following requisite fees:

Administrative Charge (Change of Address) : Rs. 200.00
Administrative Charge (Change of Phone Nos.) : Rs. 200.00

Application for Change in Transport Details

The application form for availing transport facility or withdrawal of the same may be downloaded by the parent, filled in and submitted at the School Office. The withdrawal shall be done with immediate effect and the Transport Commitment Charges deposited shall be forfeited. However, the application for providing transport facility shall be considered after verifying the availability of seats on the concerned route. In case there is a seat the parent shall be intimated about the same and s/he shall deposit a Transport Commitment Charge of Rs. 5000.00 (non-refundable) and avail the facility.

Administrative Charge (Change of Bus Stop and/or Route) : Rs. 200.00
Administrative Charge {Change of Transport Mode - from Transport to Supervision or vice-versa) : Rs. 200.00

Application for Issuance of Transfer Certificate

The application form meant for the issuance of Transfer Certificate may be downloaded by the parent, filled in and submitted to the Principal’s PA.
